Health Systems Peer Support

Thrive’s Health Systems Peer Support programming offers immediate and impactful support to individuals with substance use disorders and/or mental health diagnoses in emergency rooms, inpatient hospitals, and other acute or skilled nursing facilities. As Ohio’s most experienced provider of embedded peer support services, we’re at the forefront of integrating peer support into healthcare settings to enhance patient care and outcomes.

Activities: The program’s activities are designed to provide comprehensive support during critical times, including:

  • Providing peer support within emergency rooms, inpatient hospital settings, and other healthcare facilities to individuals presenting with mental health or substance use challenges.
  • Working closely with medical and psychiatric staff to create a cohesive support network that addresses both medical and recovery needs.
  • Assisting patients in navigating the healthcare system, ensuring they understand their treatment options, and helping them make informed decisions about their care.
  • Facilitating smooth transitions from acute care settings to community-based support or other levels of care, aiming to prevent readmissions and promote sustained recovery.
  • Connecting individuals with community resources and supports, such as outpatient services, housing, employment, and peer support groups, to support their recovery journey beyond the healthcare setting.
