HIPAA & Consent Form

The HIPAA notice describes how mental health and medical information may be used and disclosed and how you can get access to this information. The consent form explains the services offered at Thrive and documents the client’s consent to receiving that treatment. This form must be completed to receive Thrive services.

CHESS Health Connections

Connections e-Recovery app, is offered by Thrive Peer Recovery Services in conjunction with Chess Health. This grant supports people with a history of substance use disorders with digital recovery supports and local services, providing access to a recovery community, evidence-based cognitive behavioral therapy modules, and information on local recovery-related resources.
