A Path to Thrive is a comprehensive, evidence-based peer support model developed from more than seven years of experience providing peer-led interventions in emergency departments. Unlike other ER-initiated peer support programs, A Path to Thrive delivers a fully integrated and sustained continuum of care designed to address both immediate and long-term recovery needs. The program has demonstrated measurable success in reducing hospital readmissions and improving HEDIS quality measures through structured, data-driven engagement. By combining lived experience, clinical collaboration, and proven outcomes, A Path to Thrive represents a new standard in peer support innovation and healthcare transformation.
